Community Transition Team

The Community Transition Team is critical to increasing the quality of life for some of the most vulnerable members of our community.

By assisting individuals living with mental illness in navigating the various support systems in Albany County, we help patients at the Capital District Psychiatric Center transition to community living by securing supportive housing, mental health treatment, primary care, disability benefits, and employment.

Building the Future

We provide support for young adults leaving the children’s mental health system by creating connections to adult mental health services such as housing, vocational training, and primary care, as well as setting and accomplishing education and employment goals.

Making an Impact

Annually the Community Transition Team helps over thirty people living with severe and persistent mental illness gain independence and a higher quality of life through referral-making, advocacy, teaching, and support.
